This is a Scrolling Widget, which shows call/sms history.
User can tap on the call/SMS icon to view the details and call back/reply.

Please make sure scrolling widget option is enabled in your Launcher application. ****************
TO ENABLE : Long tap on your home screen and select 'Call & SMS Log'
